    Since HUF PPF account are not eligible for extension of period, they are not eligible for any interest on that.

    Visit SBI branch with necessary documents like your PPF a/c details, photo id, PAN card, address proof, photo etc. and get the required forms from SBI. The following link will also help you.

    Carry your documents like PAN card, photo, address proof etc. to your bank / post office and request them for same. They will provide you the required form.
